Registration

To obtain and guarantee a weekly lesson at the Musical Arts Institute there is a $25.00 (twenty-five dollars) Registration Fee.  Registration will be collected before the first scheduled lesson. This is a one-time, non refundable fee.  Visa and MasterCard credit cards can be accepted over the phone to pay for registration and secure your schedule.   

 

There are four alternative ways to begin your registration process:

•  PHONE IN REGISTRATION establish your music class schedule;

•  IN-PERSON REGISTRATION come in to our business office located at 9244 S. Lafayette in Chicago, IL 60620.  Our Office hours are Tuesday-Friday 10:00am-6pm and Saturdays 9:00am-2:00pm

• ONLINE REGISTRATION FORM fill out and submit via E-mail to Musical Arts Institute and we will call you to arrange a class schedule.  Please submit your Registration payment along with your online application.  Click here to pay registration fee.

•  PRINT FRIENDLY REGISTRATION FORM print, fill out, and mail to us and we will call you to arrange a class schedule and you can bring the registration form with you when you come in for your first scheduled lesson.  Please include the $25.00 registration fee with your registration form.
